Cases of another successful reperfusion therapy in the Pineo medical ecosystem:

We fight stroke despite the pandemic:

Through the Head of the Neurological Service of the Pineo Medical Ecosystem, the President of the Georgian Stroke Association, Professor Alexander Tsiskaridze, and the Interventional Neurosurgeon, Professor Mirza Khinikadze, four successful thrombectomies were performed last week. All four patients are feeling well and have already been discharged from the clinic.

Patient 1. 69-year-old woman, 12 hours after stroke. LVO – Internal carotid artery thrombosis. Clinical / Radiological Inconsistency (NIHSS / ASPECTS mismatch) -19/8, NCCT / mССT; Mean degree of collateral circulation (mCTA collateral score 3). Thrombectomy with SR technique (pRESET). Recanalization TICI2b – Good reperfusion, clinical picture improvement – NIHSS – 10.

Patient 2. A 58-year-old woman, 40 minutes after the onset of stroke. LVO – Stem / occlusion of the distal segment of the right carotid artery; NIHSS-17, ASPECTS-8. Severe anemia, cardiac postoperative period (20 days after aorto-coronary shunting) – a contraindication to thrombolysis. Good degree of collateral circulation (mCTA collateral score 4). Thrombectomy with ADAPT technique (Sophia Plus). Correction of anemia with hemotransfusion and cardion-intensive therapeutic support was performed during neurosurgery. Recanalization TICI3 – Complete reperfusion, dramatic clinical improvement – NIHSS – 0.

Patient 3. 78-year-old male, 1 hour after stroke. LVO – occlusion of the M1 segment of the left middle artery; NIHSS-6 (Eloquent Zone – Speech Center); ASPECTS-7; Mean degree of collateral circulation (mCTA collateral score 3). Combination therapy – thrombolysis with alteplase / thrombectomy with ADAPT technique (Sophia Plus); Recanalization TICI2b – Good Reperfusion, Full Functional Recovery – NIHSS – 1.

Patient 4. 83 year old woman, 1.5 hours after stroke. MeVO – occlusion of the M2 segment of the left middle artery; NIHSS-12 + Eloquent Zone – Speech Center; ASPECTS-10; Transferred after two strokes mRS-1; The date of development of the second stroke is unknown – a contraindication for thrombolysis; Mean degree of collateral circulation (mCTA collateral score 3). Thrombectomy with SR technique (pRESET Lite); Recanalization TICI3 – Complete reperfusion, significant improvement in clinical picture NIHSS -1.
